I am writing to you that you are required to attend a hearing for the possibility of terminating your employment on [insert date and time]  in [insert location of hearing]. 

The company is considering terminating your employment for the reasons below:

To the best of the company’s knowledge, there are not any additional relevant documents for this procedure; however, if you believe that there are any relevant documents that you would like to procure for the meeting, you are welcome to request them, provided that they can be located by reasonable means. 

You may bring a qualified representative to the hearing meeting on your behalf. 

You are also entitled to not appear at the hearing, and are allowed to submit your arguments, reasons, and requests, in writing by the date of the hearing. 

For clarification, your claims and requests both written and oral will be carefully considered before any decision is made regarding your employment for the company, 

If for any reason you do not appear at the hearing and/or your claims are not submitted in writing by the date of the hearing, the decision in your case will be made based on the data available in the company only. 

After any decision has been made, in regards to your case you will be sent a written notice.
